LIFE & AD&D INSURANCE
Life Insurance provides additional financial protection for you and your family in the event of your death. Dutch Valley Food Distributors provides $50,000 of life insurance for each eligible employee. Accidental Death & Dismemberment (AD&D) insurance provides additional financial protection if you are seriously injured or die due to an accident. This coverage is included with your group term life insurance. Your benefit reduces by 65% at age 70 and an additional 50% at age 75. For full details, please refer to your insurance certificate.​
SHORT-TERM DISABILITY
You are considered disabled under the Short-Term Disability benefit if a covered injury or illness keeps you from performing the main duties of your job and you are unable to work or earn a paycheck. Benefits begin right away for accidents or after 7 days for an illness, replacing about 66â…”% of your weekly income (up to $2,000 per week) for up to 13 weeks. An injury is a condition caused by an accident, and an illness is a sickness or disease.

LONG-TERM DISABILITY
You are considered disabled under the Long-Term Disability benefit if a covered condition keeps you from working for an extended period of time. Benefits begin after 90 days and replace 60% of your monthly income (up to $15,000), helping you cover everyday expenses like housing, childcare, and groceries. Benefits may continue until you reach Social Security Normal Retirement Age.

A pre-existing condition limitation applies, meaning there is no coverage during the first 12 months for conditions you received treatment for in the 3 months prior to coverage.
