VOLUNTARY LIFE INSURANCE & AD&D

Dutch Valley Food Distributors offers Voluntary Life and AD&D Insurance for you and your family. Because this is a voluntary benefit, you pay 100% of the cost—but it provides an affordable way to add extra financial protection at group rates. This coverage is portable, meaning you can keep it if you leave the company.

VOLUNTARY ADDITIONAL LIFE INSURANCE

Employee Coverage

Purchase in $10,000 increments, up to $500,000 (max 5x salary)
Evidence of Insurability required over $130,000
Benefits reduce by 50% at age 70

Spouse Coverage

Purchase in $5,000 increments, up to $500,000 (not to exceed 100% of employee amount)
Evidence of Insurability required over $30,000
Reduces at the same time and percentage as employee coverage

Child Coverage

Coverage available from birth to age 19 (or 26 if full-time student)
Purchase in $1,000 increments, up to $10,000

VOLUNTARY ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)

AD&D coverage provides an additional benefit if you or a covered dependent experience a serious injury or death due to an accident.

Employee: Up to $500,000 (max 5x salary)
Spouse: Up to 100% of employee amount (max $500,000)
Child: Up to $10,000
Coverage reduces on the same schedule as Life insurance

Life Insurance (Age-Banded)

Rates vary based on age for both employee and spouse
Child rate: $0.076 per $1,000 of benefit

AD&D Insurance

Employee: $0.020 per $1,000 of benefit
Spouse: $0.020 per $1,000 of benefit
Child: $0.051 per $1,000 of benefit

IMPORTANT NOTE

If you don't enroll when first eligible, Evidence of Insurability will be required later. Costs are deducted from your paycheck. Coverage amounts and costs adjust as you enter a new age band.
